Moving On with Housing Choice Vouchers (HCV)

Referrals for the UNITY “Moving On” Initiative are being accepted for current PSH clients in New Orleans and Jefferson Parish to transition in place to Housing Choice Vouchers.

“Moving On” is a partnership between local Public Housing Authorities and the Continuum of Care. It is designed for people who are currently enrolled in Permanent Supportive Housing (PSH) programs who:

  • Demonstrate housing stability in their housing unit;
  • Are no longer in need of intensive services; and
  • Are ready and able to move on to a Section 8 Housing Choice Voucher rental subsidy.

This iniitiative is a way for people to transition to rental assistance from the local Public Housing Authority while remaining in their housing unit to transition in place.  The initiative will create program openings and allow the CoC to refer new clients who need the supports of PSH programs.

Programs submitting an application agree to:
1) Assist client to maintain housing stability for 6 months to ensure successful transition
2) Accept a new referral from the CoC Coordinated Entry System Manager upon submission of the application to UNITY

For clients interested and eligible in “Moving On,” PSH Case Managers need to complete and submit an application packet consisting of three (3) referral documents plus attachments:

  • The “Moving On” Screening Assessment accessed here
  • The “Moving On” Assessment Tool and Referral Packet accessed here
  • The HUD 90026 form is accessed here.

Below is the application for each housing authority that needs to be submitted along with all required documents listed:

  • The Jefferson Parish Housing Service Development District (JPHSDD) application is accessed here. The new landlord packet is accessed here.

Clients who have been assessed by their Case Manager as meeting the minimum referral criteria, including their score on the assessment tool, can submit completed referral packets and Housing Authority applications to:
